Uses
Acetylcysteine is a multifunctional pharmaceutical agent primarily employed in respiratory disorders and antioxidant therapy. As a mucolytic agent, it reduces sputum viscosity by cleaving disulfide bonds in mucoproteins, making it clinically indicated for patients with chronic bronchitis, emphysema, and cystic fibrosis, typically administered via nebulization or oral formulation. In the emergency management of acetaminophen (paracetamol) poisoning, NAC serves as the standard antidote, preventing hepatic necrosis through glutathione replenishment when administered intravenously within 24 hours post-ingestion. Furthermore, its antioxidant properties confer therapeutic benefits in cardiovascular surgery (mitigating myocardial ischemia-reperfusion injury), neuropsychiatric disorders (as adjunctive therapy for obsessive-compulsive disorder and bipolar disorder), and nephroprotection (preventing contrast-induced nephropathy). Recent studies have also demonstrated NAC's potential antiviral effects (against influenza and HIV) and metabolic benefits (e.g., in polycystic ovary syndrome).
